ABSTRACT

Objective:To investigate and analyze the correlation between blood pressure and serum creatinine (Scr) and glomerular filtration rate (GFR).Methods:This research was a cross-sectional study, a total of 46792 people over 20 years old with relatively complete data in the health management center of the First Affiliated Hospital of Xinjiang Medical University from January to December 2018 were included as the research subjects, and baseline data of these study subjects were obtained by on-site questionnaire, collection of blood pressure, creatinine and other physiological and biochemical indicators. The subjects were divided into 20-64 years old group and ≥65 years group according to their age; being treated as the continuous variables (systolic blood pressure, diastolic blood pressure, five categories), their blood pressure indicators were analyzed. The F test, t test, χ2 test and logistic regression model were used to analyze the baseline characteristics of Scr level and GFR, as well as the correlation between blood pressure and Scr and GFR. Results:(1) The average Scr level was (72.89±16.84) μmol/L in the study population aged 20-93. The average GFR was (102.17±17.01) ml·min -1·(1.73 m 2) -1. The average Scr of men was higher than that of women, the average GFR of men was lower than that of women (all P<0.001). (2) Among the subjects above, there were 514 cases (1.10%) with abnormal Scr level and 657 cases (1.40%) with abnormal GFR, the incidence of abnormal Scr and GFR was higher in men, subjects with age ≥65 years old, or diabetes, or hypertension (all P<0.05). (3) There were significant differences in Scr level and GFR in subjects with different gender, age, blood pressure and body mass index (BMI) (all P<0.05). (4) With the increase of blood pressure, among the crowd of 24-64 years old, the relative risk of abnormal Scr and GFR in the group with baseline blood pressure ≥180/110 mmHg was 8.434 and 5.168 times higher than that in the group with baseline blood pressure<120/80 mmHg, respectively, after adjusting variables such as sex, age, nationality, BMI, hypertension, diabetes. However, among the crowd of ≥65 years old, only the relative risk in the group with baseline blood pressure of 160-179/100-109 mmHg was statistically significant compared with<120/80 mmHg. Conclusion:The blood pressure is bound up with Scr and GFR in the crowd of 24-64 years old.

ABSTRACT

Objective This paper analyzed the conclusion report of physical examinations by the chief inspection physician from the Health Management Center,The First Teaching Hospital of Xinjiang Medical University in Urumqi,to examine the quality of these reports.The research aimed to provide a reference for improving the quality of the chief physician's conclusions from physical examinations.Method Using the method of blinding,three senior physicians as quality control doctors analyzed the value of concordance of 12 489 conclusions from physical examinations by seven chief inspection physicians made from September to October 2017 based on three aspects,including standardization of classified disease-diagnosis handwriting,conclusions of physical examination and sequence of positive results,and unified diagnosis.Results The results showed that value of concordance of standardization of classified disease-diagnosis handwriting of the reports by the seven chief inspection physicians was 95.2%-96.5% and of conclusions of physical examination and sequence of positive results was 94.0%-96.1%.Additionally,these two values indicated no statistically significant differences among the seven chief inspection physician (x2=9.906,9.740respectively,P>0.05).However,the concordance value of unified diagnosis by the seven chief inspection physicians was 86.2%-94.6%,displaying statistically significant differences among the chief inspection physicians (x2=92.990,P<0.001).Conclusion It was concluded through the research that differences in the chief inspection physicians' reports of physical examination reached normalization and unification in aspects of standardization of classified disease-diagnosis handwriting,and conclusions of physical examination and sequence of positive results.However,statistical variations in aspects of unified diagnosis revealed that training should be conducted for the chief inspection physician for physical examination.

ABSTRACT

Objective Analysis of community work personnel's general practitioner engaged in service intention and its influencing factors. Methods Between December 2016 and May 2017, using multi-stage cluster random sampling in the Urumqi area,across 7 community health service centers(CHSC) out of 59 in a county jurisdiction,we randomly selected 1-2 community health service centers in each area county.Wethen randomly selected 8 CHSC staff as the research participants and chose a total of 324 community work personnel, 66 male and 258 female, with an average age of (34.7 ± 10.4) years to complete a questionnaire survey.The general practitioner engaged with theservice staff will use chi-square test analysis and logistic regression analysis on the influencing factors. Results Of 324 medical workers in community health service centers, including 250 of Han ethnic group and 74 minority ethnic group workers, 97.6% workers of Han ethnic group and 95.9% of minority ethnic group had signing service awareness; 92.4% of Han workers would sign system services,and 95.9% of the minority ethnic group staff were willing to sign system services. Single factor analysis found that the staff were familiar with the signing system service, attended the health management training in general medicine,had teacher-training background,had teaching experience,wondered whether to have interns,wondered whether it can save manpower,wondered whether workload could be reduced, wondered whether it can improve compliance, and considered doctor-patient relationship. These nine parameters represented service contracts with the GPS system and showed statistically significant differences (χ2=6.433, 12.913, 5.984, 6.448, 14.593, 20.675, 17.031, 84.829, 97.943, respectively;P< 0.05). In the comparison of Han ethnic group and minority workers' signing intention,the nine indicators showed no significant differences(P>0.05).Conclusion Urumqi Han ethnic group and minority community workers' a wareness of signing service, willingness to sign system services, strengthening community staff of general medicine,and training for health management knowledge and skills are advantageous to the community engaged in service.

ABSTRACT

Objective To explore the relationship between glycemic controls with periodontal diseases and disease conditions in patients with type 2 diabetes mellitus (T2DM).Methods Glycated hemoglobin A1c(HbA1c) and serum monocyte chemoattractant protein 1 (MCP-1) levels were measured in 34 normal subjects (control group) and 85 T2DM patients.The patients were assigned to four groups according to MCP-1 levels and complicating periodontal diseases.Both Pearson correlation analysis and univariate Logistic regression analysis were used.Results The levels of HbA1c and MCP-1 in T2DM group were significantly higher than those in control group (P <0.01);and the levels of HbA1c and MCP-1 in patients with periodontal disease were significantly higher than those without periodontal disease (P < 0.01).In both normal MCP-1 with periodontal diseases and elevated MCP-1 without periodontal diseases groups,HbA1c level was significantly higher than that in normal MCP-1 without periodontal diseases group (P < 0.01).In elevated MCP-1 with periodontal diseases group,patients had significantly higher HbA1 c levels than that in either periodontal diseases group or elevated MCP-1 group,respectively (P < 0.01).In HbA1c≥7.0% group,MCP-1 levels and the incidence of periodontal diseases were significantly higher than that in HbA1c <7.0% group (P <0.01).HbA1c levels were positively correlated with MCP-1 levels in HbA1c≥7.0% group (R2 =0.6004,P < 0.01).Univariate regression analysis showed that HbA1c ≥7.0% was the risk factor for complicating periodontal diseases in T2DM patients (OR =1.65,95% CI:1.12 ~ 2.56,P < 0.01).Conclusions Poor glycemic control is related to periodontal disease and the disease conditions in T2DM patients.Elevated HbA1c levels can be used as the assessing indicator for periodontal diseases of T2DM patients.

ABSTRACT

Effective expression of pIFN-α in recombinant Pichia pastoris was conducted in a 5 L fermentor. Ethanol accumulation during the late glycerol feeding period inhibited heterologous protein expression. Comparative transcriptome analysis was thus performed to compare the gene transcription profiles of Pichia pastoris KM71H in high and low ethanol concentration environments. The results showed that during the glycerol cultivation stage, 545 genes (265 up-regulated and 280 down-regulated) were differentially expressed with ethanol stress. These genes were mainly involved in protein synthesis, energy metabolism, cell cycle and peroxisome metabolism. During the methanol induction stage, 294 genes (171 up-regulated and 123 down-regulated) were differentially expressed, which were mainly related to methanol metabolism, amino acid metabolism and protein synthesis. Ethanol stress increased protein misfolding and reduced structural integrity of ribosome and mitochondria during cultivation stage, and led to the failure of endoplasmic reticulum stress removal and damaged amino acid metabolism during induction stage in Pichia pastoris.

ABSTRACT

Objective To observe the method of mechanical ventilation in the chest compressions during cardiopulmonary resuscitation (CPR),and to explore the influence of the flow pattern selection of square-wave and decelerating-wave on airway pressure of patients. Methods A prospective self-pairing study was conducted. Forty patients undergoing CPR admitted to Department of Emergency of Lishui City Central Hospital from January 2011 to February 2013 were enrolled. Using Respironics Eisprit ventilator,the working mode and parameters of ventilator were set reasonably according to previous research,while the chest compressions was performed in a stable state by the same doctor,. Each patient received different flow,waves,including square-wave and decelerating-wave,and the highest peak airway pressure was recorded as a pair of data when the time-pressure and time-flow waveform were frozen. Two pairs of data by different doctors were collected in each patient. Eighty pairs of data from 40 patients were collected for statistical analysis by paired t test. Results The highest peak airway pressure of decelerating-wave was (38.15± 5.99)cmH2O (1 cmH2O=0.098 kPa),which was (5.71±1.98)cmH2O lower than that of square wave 〔(43.86± 6.68)cmH2O〕with significantly statistical difference(t=22.010,P=0.000). 73.75%patients undergoing square wave with peak airway pressure over 40 cmH2O were found,but only 45.00% patients were found in decelerating-wave. Conclusion Because decelerating-wave used in mechanical ventilation during CPR can obviously reduce the peak airway pressure,the occurrence of barotrauma,and the probability of triggering high pressure ventilator alarm,and improve the compliance of ventilator,so decelerating-wave is more reasonable than square-wave.

ABSTRACT

Objective To assess the diagnostic value of bone scanning and bone alkaline phosphatase (B-ALP) detection in diagnosis of osseous metastases in lung cancer. Methods 58 patients with lung cancer were examined by bone scanning and serum alkaline phosphates(ALP) and B-ALP detection. The results were analyzed. Results Among the 58 patients, bone scanning found osseous metastases in 33 patients, the incidence was 56.90 %. Among the 58 patients, the incidence of bone metastasis of adenocarcinoma was 70.97 % (22/33) and squamous carcinoma was 42.86 % (9/21) (x2 =4.109, P =0.0427). The incidence of bone metastases increased in B-ALP ≥150 U/L. B-ALP was significantly correlated with the number of metastatic loci in bone. The sensitivity of bone scanning is the highest (93.94 %), but specificity the lowest (92.00 %).The sensitivity combining bone scanning and B-ALP is the lowest (57.58 %), but the specificity the highest (100.00 %). Conclusion Whole-body bone scanning has high sensitivity in early detection of bone metastases. For diagnosis of osseous metastases in patients with lung cancer, bone scanning is the primary choice. Combining the bone scanning with B-ALP can increase specificity. B-ALP has certain using value to confirm bone metastasis and decide the disease extent.

ABSTRACT

Curdlan is a water insoluble exopolysaccharide produced by Alcaligenes faecalis under nitrogen-limiting conditions. After excretion, the polysaccharide is attached the cell wall. Thus enhancement of biomass production during the cell growth phase is important to curdlan production. A strategy of increasing nitrogen source to improve biomass production was adopted for curdlan production by Alcaligenes faecalis (ATCC 31749). In the batch fermentation of curdlan, a relatively higher NH4Cl level of 3.6 g/L with continuous glucose feeding increased the cell density leading to improvement of curdlan production. However, excessive NH4Cl would inhibit curdlan production and biomass production was not improved significantly. In addition, feeding of ammonia water at the initial phase replaced NaOH solution to control pH at 7.0. Subsequently, feeding of NaOH solution was resumed to control pH at 5.6 for curdlan production after ammonia was consumed. As a result, biomass production and curdlan yield were both enhanced remarkably. Feeding of ammonia water during the first 24 h led to biomass production of 18.8 g/L. However, higher cell density did not lead to increase in curdlan production. The maximum curdlan production (72 g/L) was obtained by feeding ammonia water for the first 14 h, during which the cell density was about 11.9 g/L.
